Cloud has two real drawbacks.

You need working internet. If the connection drops, a cloud system is unreachable, while a server-based one keeps running on the local network. That matters if your area has patchy connectivity. For most practices, a second connection or a mobile failover costs less per month than the IT support line on a server setup. Plan for it either way.

Subscriptions never stop. A server has a large upfront cost and then feels free until it needs replacing. A subscription is visible every month, forever.
