What is a PIMS? Veterinary practice management software explained

PIMS stands for practice information management system, the software that runs a veterinary practice. What it does, what it costs, and how to choose one.

If you work in veterinary practice, you use a PIMS every day, even if nobody's ever told you what the letters stand for.

A PIMS (practice information management system) is the core software a veterinary practice runs on. It holds patient records, appointments, clinical notes, invoices and client details in one system, so the whole team works from the same information. You'll also hear it called a PIMS (practice management system). They're the same thing.

What does a veterinary PIMS do?

A PIMS is the system of record for the practice. Everything that happens to a patient, and everything the practice charges for, goes through it.

The core jobs:

  • Patient records. Clinical history, vaccinations, weights, lab results, imaging and notes for every animal, in one file the whole team can see.
  • Appointments. The diary for every vet and nurse, from routine boosters to surgical slots, with reminders that cut no-shows.
  • Clinical notes. Consult write-ups, ideally structured so the next vet can find what they need in seconds.
  • Billing and invoicing. Every service, medication and consumable priced and added to the client's invoice, then tracked until it's paid.
  • Client communication. Reminders, results, follow-ups and marketing, sent by text, email or app.
  • Inventory. Stock levels, expiry dates and reordering for medications and consumables.
  • Reporting. Revenue, appointment volumes, outstanding debt and clinical metrics, pulled from the data the system already holds.

In older setups, several of these jobs live in separate tools: one system for records, another for reminders, a card machine that doesn't talk to the invoice, a spreadsheet for stock. A practice running that way re-enters the same information several times a day. The direction of the market is consolidation: one system doing all of it, with the data in one place.

PIMS vs PIMS: is there a difference?

There's no difference. PIMS (practice information management system) and PIMS (practice management system) are two names for the same category of software. Vets, vendors and consultants use them interchangeably, and which one you hear mostly depends on where you trained and which vendor you've used. In the US, PIMS is slightly more common in formal writing; in the UK, PIMS dominates in conversation.

If a vendor uses one term and not the other, it's a naming preference, not a difference in what the software does. The same applies to variations like "veterinary information management system" or "vet practice software." Under every name, it's the same job: the system the practice runs on.

The three generations of veterinary PIMS

Veterinary software has been through three distinct eras, and all three are still being sold today. Knowing which generation a system belongs to tells you more about what it'll be like to live with than any feature list.

Server-based systems run on a physical server inside the practice. This was the only model for roughly two decades, and several of the most widely installed systems in the world are still built this way. The practice owns and maintains the hardware, updates are installed manually, remote access is limited, and if the server fails, the practice stops.

These systems are somewhat reliable, and that reliability is the whole pitch. There is no innovation happening on them. Most are dormant: the vendors collect support fees, patch what breaks, and ship little else, because meaningful improvement would mean rebuilding products that date back decades. A practice choosing one today should expect the software to work roughly as it does now for as long as they run it. Nothing more is coming.

Cloud-based systems moved the software out of the building. The system runs on the provider's infrastructure and the team logs in through a browser, from the practice, from home, or on the road. Updates and backups happen automatically. Most of these platforms were built in the 2000s and 2010s, and at the time they were a genuine leap.

Twenty years on, many of these vendors have gone dormant too. The rate of innovation across this generation has dropped considerably: release notes that once listed new features now list bug fixes, and reviewers on several platforms describe products that haven't materially changed in years. The problem is structural. The architecture these systems were built on has a ceiling, because it was designed long before modern AI existed. Supporting AI properly would mean rebuilding the foundations, and few vendors are willing to pay for that. So most bolt on a standalone tool, or nothing at all, and the gap between them and the AI-native platforms widens with every release cycle.

What should a modern PIMS include?
